Litter box problems represent a significant welfare issue for cats — elimination outside the box is invariably a sign of unmet needs or medical problems, not deliberate misbehavior. Cats who eliminate inappropriately are often surrendered to shelters, where the stress of the shelter environment may worsen the problem. The causes are usually identifiable and addressable: box size too small for comfortable posture, litter scent or texture aversive, location in a high-threat or inaccessible area, insufficient boxes in multi-cat households, or underlying medical conditions causing pain or urgency. A welfare-centered approach treats inappropriate elimination as a communication requiring investigation rather than a behavior requiring punishment. Punishment worsens anxiety and the problem significantly.
