Systemic lupus erythematosus (SLE) in dogs is a rare but serious immune-mediated condition causing multi-organ inflammation and requiring sustained immunosuppressive therapy with significant welfare implications.
SLE creates a sustained welfare management challenge because the disease itself, the diagnostic process, and the treatment all impose welfare costs. Affected dogs experience a constellation of painful, debilitating signs across multiple body systems — joint inflammation causing reluctance to move, skin ulceration causing discomfort and grooming difficulties, and haemolytic anaemia causing fatigue and weakness. The diagnostic journey involves multiple tests, veterinary visits, and often hospitalisation before the multi-criteria diagnosis is confirmed. Long-term immunosuppression is effective but not without cost: high-dose corticosteroid side effects (muscle wasting, increased infection risk, behavioural changes) affect quality of life, and steroid-sparing agents like azathioprine carry bone marrow suppression risks requiring regular blood monitoring. The goal of management is the lowest effective immunosuppressive dose that controls disease while minimising treatment side effects. Regular quality-of-life assessment using owner-completed scales helps ensure that treatment serves the animal's welfare rather than simply extending life at the cost of comfort.
