The welfare implications of different hyperthyroidism treatments differ significantly. Medical management with methimazole or carbimazole requires twice-daily tablet administration for life — a welfare challenge for cats who resist handling, and associated with side effects requiring ongoing monitoring. Radioiodine provides permanent cure without ongoing medication but requires a period of hospitalisation-like isolation that is stressful for some cats. Surgery carries anaesthetic risks in older cats, though experienced thyroid surgeons achieve excellent outcomes. The iodine-restricted diet works but requires perfect compliance. For the cat, the welfare comparison centres on the chronic burden of long-term medication versus the acute stress of radioiodine isolation, balanced against the individual cat's tolerance for handling and medication. For many cats, radioiodine's permanent cure with no ongoing medication represents the best welfare outcome, once the isolation period is managed with appropriate enrichment and comfort measures. The concurrent renal disease complication adds welfare complexity: successfully treating hyperthyroidism may unmask renal failure that was previously subclinical, requiring ongoing monitoring and management of the newly apparent kidney disease.
