Feline idiopathic cystitis (FIC) is the most common cause of lower urinary tract signs in cats. Its strong link to stress and environment makes it both a medical and a welfare management challenge.
FIC is a welfare condition where the environment is both cause and cure. The pain of cystitis without infection — inflammation driven by stress neurochemistry causing bladder mucosal damage — responds poorly to antibiotics (which are often inappropriately prescribed) but significantly to stress reduction and environmental modification. Cats experiencing recurrent FIC are often living in environments with insufficient resources (litter trays, resting spaces, vertical height) or ongoing social conflict. The welfare imperative is to treat the acute pain while addressing the underlying environmental drivers. Urethral obstruction in male cats is a welfare emergency — owners of male cats with FIC should be educated about the signs of obstruction and the urgency of immediate veterinary care.
