Cushing's disease and diabetes mellitus share clinical signs but require very different management. Accurate diagnosis prevents treatment errors that prolong suffering in affected dogs.
The welfare significance of distinguishing Cushing's from diabetes lies in the treatment implications. A dog receiving incorrect treatment continues to suffer from the untreated condition while potentially experiencing iatrogenic harm from inappropriate intervention. Both conditions cause significant chronic suffering when inadequately treated: the hyperglycaemia of uncontrolled diabetes causes progressive complications; the hypercortisolaemia of Cushing's causes multi-system deterioration. Accurate diagnosis is the gateway to effective treatment and welfare improvement. In dogs with concurrent conditions, staged treatment — typically addressing Cushing's first to assess whether diabetes resolves — requires careful veterinary judgement and owner communication.
