Sarcoid management decisions require welfare consideration of both the condition and the treatment. Small, stable occult sarcoids in non-problematic locations may warrant monitoring rather than treatment — the welfare cost of aggressive treatment exceeds the welfare cost of the sarcoid itself. However, fibroblastic and mixed sarcoids in locations that interfere with equipment fitting, cause discomfort during work, or are susceptible to trauma create genuine welfare burdens that require active management. The welfare-centered approach evaluates each sarcoid on its clinical behavior, location, and trajectory rather than applying a uniform treatment philosophy. Early treatment of progressing sarcoids consistently achieves better outcomes with less welfare cost than treating advanced disease.
