Navicular syndrome has historically been a broad diagnostic category encompassing multiple distinct conditions that only advanced imaging can differentiate. Horses incorrectly labeled as navicular cases may receive inappropriate treatment, leading to ongoing pain and welfare compromise. The availability of standing MRI has been transformative, allowing precise diagnosis without the risks of general anesthesia while providing information that guides targeted treatment. When horses receive diagnosis-appropriate therapy — whether that is corrective shoeing, tendon rehabilitation, targeted injections, or surgical intervention — welfare and performance outcomes are substantially better than with empirical treatment.
