The MRI-guided approach to navicular syndrome represents a genuine welfare advance: instead of treating a syndrome, veterinarians can now identify the specific tissue pathology driving pain and target treatment accordingly. A horse with navicular bone marrow edema responds differently to treatment than one with DDFT lesions or collateral ligament damage, and treating them the same way was producing inconsistent outcomes. Bisphosphonate therapy has shown meaningful pain reduction in appropriate cases, offering a medical treatment that complements — rather than replaces — corrective shoeing and physiotherapy. The welfare implication is that horses correctly diagnosed with MRI can receive targeted treatment that achieves better pain control, longer working life, and higher quality of life than the historical approach of graduated shoeing and palliative management alone.
