Pedal bone rotation causes intense, chronic pain from the bone impinging on the sole and distorting hoof capsule anatomy. Horses with significant rotation may have bone visible through the sole in severe cases — a welfare emergency. The chronic pain of unmanaged rotation significantly reduces quality of life, and many horses with severe rotation are ultimately euthanised when pain cannot be adequately controlled. Prevention through proactive laminitis management — identifying at-risk horses, restricting grass access, managing metabolic conditions, and responding rapidly to early signs — avoids the welfare catastrophe of rotation entirely. Regular radiographic monitoring in horses that have had laminitis catches early rotation before it becomes severe.
