Cats with megacolon experience chronic colonic discomfort, periodic obstipation crises requiring hospitalization and manual deobstipation, and the indignity and pain of repeated straining. The welfare impact is significant and continuous. Medical management can control mild cases, but true megacolon eventually requires surgical intervention. Sub-total colectomy has excellent welfare outcomes - most cats have normal fecal consistency and urgency within weeks and experience dramatically improved quality of life compared to years of medical management. Earlier surgical referral rather than prolonged medical management often serves welfare best.
