Autoimmune diseases cause genuine pain and systemic illness. Pemphigus causes skin pain and crusting that impairs normal behavior; polyarthritis causes joint pain that limits mobility; systemic lupus causes multisystem illness that profoundly reduces quality of life. Management with immunosuppression is necessary but carries its own welfare trade-offs. The goal is maintaining the best possible quality of life with the lowest effective immunosuppressive burden, monitored regularly and adjusted based on welfare assessment.
