Welfare-centred management of perianal fistulas has been transformed by cyclosporin (Atopica). Previously requiring aggressive surgery with high recurrence rates, cyclosporin induces remission in most cases. Tacrolimus ointment (topical) is used adjunctively. Surgical debridement of non-healing tracts may be needed. Dietary modification with hydrolysed or novel protein diets reduces immune stimulation. Long-term suppressive therapy maintains remission. Regular welfare assessments monitor comfort and quality of life.
